Changes to the 0800 number in Participant Information Sheets
The HDEC templates that previously contained the old Ethics contact phone number have been updated. Instead, these now include the Ministry of Health general enquiries line for participants who have limited internet access but would still like to contact the HDECs with their ethical concerns or complaints.
The updated templates include:
- Main Participant Information Sheet and Consent Form Template
- Pregnancy Follow-up (Child) Information Sheet and Consent Form Template
- Model Patient Information Sheet and Assent Form (for patients 7–11 years old)
- Partner Pregnancy Information Sheet and Consent Form Template (April 2023)
- Model Patient Information Sheet and Assent Form (for patients 7–11 years old)
Researchers are encouraged to make the same adjustments to their participant information sheets, or otherwise advise participants they can contact the Ministry of Health if they are unable to email the HDECs.
